After School Transport Change Form

Parent/guardian can complete this form to request a change in dismissal routine. All online requests should be made by 12:00 p.m.  For any changes or emergencies after 12:00 p.m., please contact the office at 703-326-3100.

  • Form response will be forwarded to the classroom teacher indicated.  If you have multiple Herndon Elementary School students for whom this change applies, you must complete this form for each child. 
  • When a student is going home with another student, it is expected that parents have coordinated plans beforehand.  Each family must submit the online form by 12:00 p.m.
  • Bus seats are limited.  Limit requested bus pass to one additional bus rider.  Please note that due to COVID-19 FCPS Health Guidelines, bus drivers may refuse additional riders based on capacity.
  • Reminders: We ask you to communicate with your child(ren) about dismissal plans BEFORE their morning arrival.
This form can only be filled out by the student's legal guardian.
